Scenes with Dr. Sarah Reese 59

[observes Sarah Reese putting a central line into a dummy] I can't tell you how many times I missed before I finally got it.

Sarah Reese

I know how to do it. I just couldn't do it in there.

It takes practice, that's all.

Sarah Reese

I'm fine with him. I never miss.

Yeah, he's the ideal patient. You can't hurt him, and his life's not hanging in the balance.

Sarah Reese

If this rotation weren't a requirement, I wouldn't be here.

Where would you be?

Sarah Reese

I'm a lab person.

Pathology?

Sarah Reese

Maybe.

Every med student, once they start dealing with patients, thinks they'd do better in Pathology. I did.

Sarah Reese

I got Parker's test results back.

Sarah Reese

You need to take a look. Here.

CT negative. We need to order...

Sarah Reese

An MRI, I know. I already did it. It shows excessive demyelination.

Explains the clumsiness, degeneration of motor skills...

Sarah Reese

And the seizure. I don't want to jump to conclusions, but I'm pretty sure he has...

Krabbe disease.

Sarah Reese

Yeah. He's eight, so it's late-onset.

Gives him five to seven years before he becomes confined to a bed and completely vegetative. [sighs] Poor kid.

Sarah Reese

What do you want to do?

Well, I've got to go talk to the par...

Sarah Reese

Dr. Manning? You okay?

No. I think my water just broke.

[looks down at the floor and sees a huge puddle forming from her legs]

Oh my God. It did. I guess this is happening today.

Sarah Reese

Uh, can we get a wheelchair over here?

Look, I'm sorry, Sarah, but you have to go tell the parents.

Sarah Reese

What, me? No, I can't...

You have to.

Sarah Reese

I don't have the experience.

Go get Dr. Charles. He will help you. I have to go have this baby now.

[sits down in a wheelchair]

Thank you.

Sarah Reese

[nervously] Okay.

[after Reese caught the brunt of a collapsing patient] Anything hurt?

Sarah Reese

Not really. Gonna be stiff tomorrow, though, aren't I?

Yeah, but pain builds character.

Sarah Reese

According to who, exactly?

Bret still needs an LP. You good?

Sarah Reese

Uh... fourth period, bottom of the ninth... put me back in, coach.

Butchered hockey, football, and baseball in one sentence. Impressive.

Sarah Reese

Stop trying to calm me down. You're not good at it.

JTJoey Thomas

I'm just saying there's no reason to be nervous.

Sarah Reese

Joey, it's match day. They're gonna hand me a piece of paper telling me where I'm doing my residency.

JTJoey Thomas

I understand.

Sarah Reese

It's literally the turning point in my life, both the world's assessment of my past and a map of my future. It's my whole life.

JTJoey Thomas

That's an awfully dramatic way to put it.

Sarah Reese

That's because you're not good at calming me down.

PPatient

Nurse? Nurse? Hey, nurse!

Sarah Reese

Are you talking to me?

PPatient

Hello? Been here for two hours with snot pouring out my nose.

Sarah Reese

I'm sorry, but we need to see patients in order of urgency.

PPatient

And I'm dying of a sinus infection. Maybe if you got off that iPad and did a little work, things would go faster.

Sarah Reese

Maybe you just need to wait your turn. And I'm not a nurse.

Handled like a true pathologist.

Sarah Reese

I know, I'm sorry.

My match day, I threw up six times and passed out, so I'd say you're doing fine.

Sarah Reese

You did not throw up and pass out.

Wanted to, though.

Sarah Reese

Was emergency medicine your first choice?

My only choice. All I ever wanted to do.

Sarah Reese

[referring to an obnoxious patient] Even though it comes with a lifetime supply of angry snot guys?

Has its downsides. Then again, how many jobs are there where you literally get to save lives?

What's happening?

Sarah Reese

Dr. Perrington is taking him to the cath lab for a clot buster. If you're right about Andrew's head traumas, giving him a clot buster is worse than giving him blood thinners.

I gave my medical opinion. The patient and his brother did not want to listen. Perrington's an attending. It's out of my hands now.

So, Dr. Reese, the reason I'm here... uh, aside from the coffee of course, is that Med has a residency opening for you.

Sarah Reese

I'll take it.

On my service.

Sarah Reese

Psychiatry?

Enough foam. Enough foam.

Sarah Reese

Okay. Um... Dr. Charles, thank you, but I've never considered psychiatry. I don't think I'm equipped.

I think you have all the tools. You're a problem solver. You pay attention. And look at how beautiful you're making my coffee despite all my meddling.

Sarah Reese

But... I don't know how to talk to people.

It's actually more about listening than talking.

Sarah Reese

[handing him a coffee cup] Grande quad three-quarters-caf nonfat, no whip mocha.

I'll tell you what. How about you come and spend the day with me, and then see what you think.

You know who the most unhappy people in the world are?

Sarah Reese

Hmm?

Fifteen year old girls.

Sarah Reese

I remember.

Mom said Emmie had hundreds of friends. Hyperbole?

Sarah Reese

Maybe not. [showing him her phone] She could have hundreds of friends.

Oh, like that.

Sarah Reese

Girls her age share everything. They judge everything. Maybe Emmie was shamed, and that led to a suicide attempt?

Let me know what you find.

Dr. Sarah Reese

And Emmie does have hundreds of friends. 623 to be exact. She's on a lot of social media platforms.

623 friends? Yikes. Who do you invite to the birthday party?

Dr. Sarah Reese

Mm-hmm. She isn't always very nice to the other girls.

Not very nice?

Dr. Sarah Reese

She wasn't shamed. She's the shamer.

Interesting. Well, so what do you think?

Dr. Sarah Reese

Maybe... Emmie realized she really hurt someone and she felt guilty?

Guilty enough to kill herself?

Dr. Sarah Reese

Hmm. I don't know.

You know what? We got to get Emmie to open up. Next time mom takes a break, jump in there, give it another shot.

Dr. Sarah Reese

Uh, Dr. Charles, wait. Where are you going?

Well, I just thought Emmie might be more comfortable talking to a woman, and somebody her own age.

Dr. Sarah Reese

But I don't know what to say, what to ask.

Well, I think that once you get in there, you'll discover that you do.

[notices Dr. Reese looking at her] What are you thinking?

Dr. Sarah Reese

Anxiety is a powerful emotion. Would you say that you feel more nervous about the situation or fearful of the future?

I-I don't know. Both, I guess.

Dr. Sarah Reese

How have you been sleeping? Waking up at night? Earlier than you'd like? [April and Maggie look at each other and start giggling] What?

Oh, my God. You're shrinking me. [to Maggie, amused] Three weeks in psych, and she's Freud.

Dr. Sarah Reese

You've created a monster, you know that? Turns out the English language is primarily comprised of buzzwords for borderline personality disorders. And now that I know them, I can't have a conversation without immediately seeing the psych in people, which is clearly an impulsive reaction, which, by the way, could also suggest a borderline personality disorder. I mean, I... I... I can't even not analyze myself. And now I'm using double negatives.

You're a first year psych resident. Compulsive diagnosing is par for the course. My first year, I was convinced that a friend of mine had dissociative identity disorder because suddenly he would only listen to Euro-Disco. Really. My advice? Think less, listen more. You'll be just fine.
